Required
- Doctor of Dental Medicine (DMD) or Doctor of Dental Surgery (DDS) from ADA or CODA-accredited school
- Passed National Board Dental Examinations (NBDE)
- Current unrestricted state dental license
- One or more years of professional dentistry experience
- Two-year residency in dentistry, oral health, craniofacial sciences, or related medical science; OR two years of professional dentistry experience
- Ability to distinguish shades of color
- Demonstrated supervisory abilities
