- PharmD or Bachelor of Science in Pharmacy from accredited institution
- Current state licensure as Registered Pharmacist or eligible for reciprocation within 90 days
- Basic Life Support certification through AHA or American Red Cross if administering immunizations
- Board of Pharmacy approved course in immunization administration or certification within 90 days if administering immunizations
Nice to have
