- PharmD or Bachelor of Science in Pharmacy from accredited institution
- Current state licensure as Registered Pharmacist or eligible for reciprocation within 90 days
- Current Basic Life Support certification through American Heart Association or American Red Cross
- Board of Pharmacy approved immunization administration course or certification within 90 days if administering immunizations
Nice to have
- Board certification
