Required
- LPN/LVN, LSW, LCSW, LMSW, or equivalent license (not RN) in applicable state
- Current unrestricted license
- Nursing Diploma or AS in Nursing or related field
- Minimum 2 years experience working with individuals with chronic illnesses, co-morbidities, or disabilities in a Service Coordinator or similar role
- Ability to travel to client sites
- COVID-19 and Influenza vaccination
