What they ask for
Required
- High school diploma or equivalent
- 3+ years experience in service coordination, case management, or similar role working with individuals with chronic illnesses, co-morbidities, or disabilities
- Current, active, valid and unrestricted RN license in applicable state
Nice to have
- Associate's, Master's, or Nursing degree
- Willingness to travel to worksites and other locations as necessary
