This is a field-based case management role for nurses working with older adults and people with disabilities in Wisconsin's Family Care program. What they ask for
Required
- Associate of Science in Nursing or Bachelor of Science in Nursing
- Minimum 3 years experience with chronic illness, co-morbidities, or disabilities in case management or similar role
- Current, active, unrestricted RN license in Wisconsin
- Minimum 1 year experience with Family Care target populations
Nice to have
- Bachelor's degree in Health or Nursing
- Case management experience with older adults or individuals with disabilities
- Long-term care background
