- Master's in Social Work (MSW)
- 2+ years of social work experience

## Nice to have

- 2+ years of care management experience
- Accredited Case Manager (ACM) certification

- Current knowledge of discharge planning, utilization management, care management, and managed care reimbursement
- Knowledge of state and federal care management guidelines
- Ability to identify appropriate community resources and collaborate across disciplines
