- Active, current, full, and unrestricted Nurse Practitioner license from a US state or territory
- Current board certification in a clinical specialty (ACNP, PMHNP, FNP, ANP, AGPCNP, or GNP) from ANCC, AANPCB, or equivalent
- Four or more years of Nurse Practitioner experience with at least one year at Nurse III level or equivalent
